The name of the company is AWV plus. Please use the plus sign NOT the word plus. AWV stands for Annual Wellness Visit. This is an online Medical Questionnaire patients take in the waiting room of their doctors office. I added the plus sign because it also helps doctors uncover additional revenue sources.

Something simple but sophisticated. Clean lines. White background. Medical Wellness colors like greens and blues.
